Prior to start of participation on any sports team, a student must have a sports physical completed and the form turned into the Jr/Sr High School Office. Once on file, the student is good to participate in practices or try outs and eventually contests for that school year. The physical examination which must be conducted by a M.D., D.O., Physician’s Assistant, or Nurse Practitioner, will be considered current for the current school year if the date of the physical examination was done on or after April 15th of the previous school year.

The MHSAA requires all student-athletes and parents/guardians to read and acknowledge this form regarding concussions.
Since the 2015-16 school year, the Michigan High School Athletic Association has provided athletic participants at each MHSAA member junior high/middle school and high school with additional insurance that is intended to pay accident medical expense benefits resulting from a suspected concussion. The injury must be sustained while the athlete is participating in an MHSAA covered activity. Policy limit is $25,000 for each accident. Covered students, sports and situations follow the catastrophic accident medical insurance. Below is a link to the paperwork for this concussion insurance.
